‘Cortisol face’ is the latest viral trend on TikTok

TikTok, is known for its viral trends. What has recently sparked a conversation is the so-called ‘cortisol face’ or ‘moon face.’ 

This has users discussing and sharing their experiences with facial puffiness and rounded appearance, attributing these changes to elevated cortisol levels. As with many health-related topics on social media, the surge in interest has led to a mix of personal anecdotes, speculation, and misinformation, prompting the need for expert clarification.

To separate fact from fiction, we spoke with Neha Cadabam, senior psychologist and executive director at Cadabams Hospitals, to understand the true nature of ‘cortisol face’ and its relationship to the stress hormone cortisol. 

She explains, “Cortisol, often referred to as the ‘stress hormone,’ is a naturally occurring steroid hormone produced by your adrenal glands. It plays a crucial role in numerous bodily functions, including regulating metabolism, blood pressure, and immune response. However, when cortisol levels become elevated for extended periods, they can trigger a variety of physical changes, including the appearance of ‘cortisol face’ or ‘moon face’.”

Factors contributing to high levels of cortisol

Chronic Stress: Persistent stress keeps your body in a constant state of fight-or-flight, leading to chronically elevated cortisol levels. This can result in increased fat storage, particularly in the face, causing it to appear rounder.

Cushing’s Syndrome: This rare hormonal disorder occurs when your body produces excessive cortisol. It can be caused by adrenal gland tumours, pituitary gland tumours, or prolonged use of corticosteroid medications. One of the hallmark signs of Cushing’s syndrome is a rounded face with increased fat deposits.

Medications: Certain medications, such as corticosteroids (prednisone, dexamethasone) used to treat various inflammatory and autoimmune conditions, can significantly raise cortisol levels, resulting in a “moon face” appearance.

Other Health Conditions: Other health issues, including polycystic ovary syndrome (PCOS) and adrenal gland disorders, can also disrupt cortisol production and contribute to facial changes.

Differentiate between weight gain in the face due to stress eating and true ‘cortisol face’

While both stress eating and high cortisol levels can lead to weight gain, there are distinct differences in how they manifest. Cadabam states, “High cortisol tends to cause fat accumulation predominantly in the face (moon face) and abdomen. In contrast, stress eating typically results in a more generalised weight gain throughout the body.”

High cortisol is often accompanied by fatigue, muscle weakness, high blood pressure, easy bruising, and skin thinning. These symptoms are usually absent in weight gain caused solely by stress eating.

Most effective ways to reduce or prevent the appearance of ‘cortisol face’

Cadabam suggests the following methods:

Stress Management: Prioritise stress reduction techniques such as exercise, mindfulness practices (yoga, meditation), spending time in nature, and engaging in hobbies you enjoy. These activities can help lower cortisol levels naturally.

Healthy Lifestyle: Adopt a balanced diet rich in whole foods, prioritise quality sleep, and engage in regular physical activity. These lifestyle factors play a crucial role in regulating cortisol levels.

Treatment of Underlying Conditions: If your high cortisol levels are due to a medical condition like Cushing’s syndrome or an adrenal disorder, work with your doctor to develop a treatment plan tailored to your specific needs. This may involve medication, surgery, or other therapies.

Medication Review: If your medications are contributing to elevated cortisol, discuss alternative options or dosage adjustments with your doctor. Never discontinue medication without medical supervision.
